Referring Veterinarian Honor Code
Healing Pups Charlotte, a part of Independence Veterinary Clinic, will only provide rehabilitation services to the undersigned client and patient. Furthermore, we will not provide general veterinary care of any type to the undersigned client and patient (excluding lifesaving procedures in case of emergency).
This agreement will be in place for one year after the patient is fully discharged from rehabilitation therapy.
